How PEO Billing Works: Anatomy of the Invoice
A PEO invoice bundles four things that look like one number: the administrative fee (theirs), benefit premiums (carriers), employment taxes (pass-through you would owe anyway), and workers comp. Reading them separately is the difference between comparing quotes and comparing marketing.
The four layers of every invoice
| Line | Whose money | Negotiable? |
|---|---|---|
| Administrative fee | The PEO's revenue | Yes - here is how |
| Benefit premiums | Carrier + employee share | Plan design, within limits |
| Employment taxes | Statutory pass-through | No |
| Workers comp premium | Carrier/program | Via class codes and loss history |
Why bundling hides price
Because taxes dominate the total, two providers can quote wildly different totals while charging nearly identical admin fees - or identical totals hiding very different fees. Always request the administrative fee as its own line, per employee, in writing. That single number is the only clean comparison point; everything else is pass-through or plan choice.
Timing and true-ups
Billing runs per payroll cycle. Watch for three recurring surprises: retro adjustments when an employee's wages change after the fact; annual benefit rate true-ups; and workers comp audits that reconcile estimated against actual payroll. Each should appear as a labeled line, never folded silently into the total.
The five-minute monthly review
- Admin fee matches the agreement times headcount.
- Taxes match your prior-cycle run rates by state.
- Any adjustment lines are explained in writing.
If line items cannot be produced on request, treat that as a pricing-transparency red flag before signing - see comparing quotes fairly.

Frequently asked
Why does my invoice total differ from the quote?
Quotes typically show admin fee plus estimates. Taxes and comp trues-up with actual payroll; benefits change at renewal.
Can I get invoiced separately for services?
Some providers offer unbundled ASO-style billing - effectively an ASO. Ask during selection.
